Skin cancer affects more people worldwide than any other form of cancer. As such, skin screenings should be performed routinely as a preventive measure for common skin cancers such as basal cell carcinomas, squamous cell carcinomas, and melanomas as well as for more rare forms of skin cancer such as merkel cell carcinoma, various sarcomas, or others. Early detection and treatment of a problematic growths often increases the chances of a successful cure and can prevent further progression. Early detection may also prevent the need for more complex treatments (such as chemotherapy and radiation) that have well-known adverse side effects that may impact quality of life.
Dr Divine offers skin cancer treatments include cryosurgery, electrodessication and curettage (ED&C), surgical excision, Mohs micrographic surgery, direct chemotherapy injections, and use of topical chemotherapy creams, such as Efudex(fluorouracil) and Aldara (imiquimod).
Mohs micrographic surgery is a highly specialized and precise treatment that is widely accepted as the most effective technique for the treatment of basal cell carcinoma and squamous cell carcinomas and is becoming increasingly popular for other skin cancers as well. This surgical technique removes one tissue layer at a time and screens carefully for cancerous tissue. Mohs removes cancerous cells with the greatest precision, while sparing the maximum amount of healthy tissue, and reducing the chances of recurrence. Dr. Divine has been extensively trained in Mohs surgery and has completed fellowship training at Vanderbilt University.
You will be asked to come in for a consultation before having surgery to examine the skin cancer, obtain a medical history, review your medications, and determine whether Mohs surgery is the best treatment plan for your unique needs. The pre-op consult allows you to meet with Dr. Divine and our staff, feel comfortable, and get answers to any questions you may have. Special considerations will be made at this time and specific instructions and recommendations may be made. At the consult, the area will be photographed for medical documentation. Additional photos will be taken the day of surgery and during the various times in the healing process. This helps the doctor track your progress and make changes if necessary. If you take the blood thinner Coumadin (warfarin) you will need to have an INR drawn 24-72 hours before surgery. If your INR is too high, your surgery may have to be rescheduled.
You do not need to fast and you can take your daily medications normally unless otherwise advised by the doctor. Instead, eat a nutritious and filling breakfast to help avoid lightheadedness during the procedure. Do not drink excess coffee. You may also bring food or snacks with you to the office as well. Avoid drinking alcohol for 48 hours prior to surgery. Please shower and shampoo your hair the night before or the morning of surgery to help provide a clean environment. Please come with a clean face, without make-up. Wear clean, comfortable, and warm clothing that can be easily changed if needed. While we do our best to keep your clothes clean, occasional staining does occur, so please dress accordingly.
Quitting smoking is always encouraged for your overall health. Tobacco and other nicotine products decrease blood supply and can impair healing drastically, causing more noticeable scars and even the need for additional surgeries. Quitting is ideal, but cutting back to 1 pack or less per day for one week before and one week after can significantly increase your ability to heal.
We do advise patients to set aside most of the day for the surgery and plan on resting for the remainder of the day. Do not schedule other doctors appointments that day or plan to return to work. The average surgery time is 3-4 hours but can last 6-8 hours or greater due to large size, delicate location, or lengthy necessary stitching. You are encouraged to bring a book, newspaper or computer to help pass the time. Patients are also advised to make arrangements to have someone available to drive them home after surgery if needed.
Before the start of surgery, Dr. Divine will review the procedure’s steps with you and obtain an informed consent. If you need clarification or further answers, feel free to ask the doctors or staff at this time. If excess anxiety is a concern, you may be given medication to make you more comfortable.
Once ready for surgery, the area will be cleansed with an antibacterial solution and the area will be numbed with local anesthetic administered through needle injection. A tissue layer will then be removed by the doctor and processed in the in-facility lab. This processing normally takes 30-45 minutes and you will be asked to wait in the waiting room. The process is repeated until the removed layer no longer reveals cancerous cells under the microscopic.
After complete removal of the skin cancer, Dr. Divine will discuss repair options including straight line suturing, a flap, skin graft, or even allowing the wound to heal on its own, often with the aid of a special dressing. After the reconstruction is complete, the surgical site will be dressed and home wound care instructions will be reviewed.
Your surgical wound will require wound care during the weeks following surgery. Detailed care instructions will be provided at the office based on your unique needs and will be added to your online patient profile through our patient portal. You should plan on keeping the area covered until your stitches are removed, 1- 2 weeks after surgery. The wound left to heal naturally may require 4-12 weeks of bandaging based on their size, location, and your own bodies ability to heal. Avoid strenuous activity and bending over for the first 48 hours if you are not on blood thinners, and a complete 1 week if you are on blood thinners. Most patients report minimal pain that responds well to treatment with Tylenol or similar over-the-counter pain medication.
If sutures have been placed, our office will have scheduled you for a follow-up visit 1-2 weeks post-surgery for suture removal. Additional wound check visits may be recommended to ensure that there are no further issues with healing or complications with the wound site. A post-op skin care regimen may be recommended.
Due to an increased risk of future skin cancers in patients who have had a history of skin cancer, we advise that patients continue to follow-up regularly, often every 6 months.
Cosmetic removal involves the surgical removal of unwanted growths, such as moles or skin tags, solely for cosmetic reasons. While moles and other growths can require removal for medical reasons, most irregularities on your skin are benign, causing no harm or danger. However, certain unwanted growths can affect your confidence, impacting your daily life.
Dr. Divine, a board-certified Dermatologist, provides cosmetic surgery removal of such growths to help you boost your confidence and delight in the appearance of your skin. If you have a mole, skin tag, or other growth you would like removed for cosmetic reasons, Dr. Divine will first perform a thorough inspection to ensure the growth is benign and not skin cancer. We will then determine the best method for cosmetic removal to achieve the results you desire. We offer a variety of treatment options based on location and patient’s preferences, building individual plans for management and care.
Moles can range in color, from black or brown to a lighter tan, and can vary greatly in size, from barely visible to larger than a quarter. Moles that remain constant throughout your life and don’t cause discomfort are typically benign, with no medical reason requiring their removal. However, there are times when clients come to Divine Dermatology with the desire to remove a mole or multiple moles for cosmetic reasons.
To remove a mole, we will perform a thorough skin assessment and determine the best method for mole removal. The most common methods include shaving the mole until it is flush with the rest of your skin, freezing off the mole with liquid nitrogen, or using laser removal. We utilize local anesthetic during the procedure, so it generally is not painful.
Skin Tag Removal
Skin tags are growths of skin that stick out slightly from the rest of the skin, typically occurring in armpits, on necks, in the groin area, on eyelids, or under breasts. Though typically harmless, these small flaps of skin can be uncomfortable and unsightly.
A skin tag removal procedure is typically short and painless, with a local anesthetic used to minimize discomfort. Dr. Divine may cut or freeze the skin tag, depending on the size, type, and location of growth.
Other Unwanted Growth Removal
Dr. Divine uses similar methods to mole and skin tag removal for cosmetic removal of cysts, milia, seborrheic keratosis, angiomas, lipomas, and other benign unwanted growths. We will work with you to determine the best cosmetic surgery option for removing the unwanted growth and improving the appearance of your skin.
Common Skin Conditions

Restylane® is a clear gel formulation of hyaluronic acid, a natural sugar that is already present in your skin, and gives your skin plumpness, volume and hydration.
